Die Anaphylaxie ist die Maximalvariante einer allergischen Sofortreaktion und stellt die schwerwiegendste und mitunter lebensbedrohliche Notfallsituation in der Allergologie dar. Die intramuskuläre Injektion von Adrenalin in den mittleren anterolateralen Oberschenkel ist die Behandlung der ersten Wahl bei Anaphylaxie.

Es gibt nur wenige zuverlässige epidemiologische Studien zur Häufigkeit anaphylaktischer Reaktionen [3]. Zusammen mit der allgemeinen Zunahme allergischer Erkrankungen in der Bevölkerung haben auch anaphylaktische Reaktionen an Häufigkeit zugenommen [4, 5, 6], so z. B. von 16/100.000 Personen-Jahren 2008 auf 32/100.000 Personen-Jahren im Jahr 2014 [7]. Bei einer Gesamthäufigkeit von 42/100.000 Personen-Jahren im Zeitraum von 2001 bis 2010 wurde ein jährlicher Anstieg der Häufigkeit insgesamt um 4,3 % und bei Nahrungsmittel-induzierter Anaphylaxie um 9,8 % beobachtet [8]. Zugenommen haben insbesondere die nahrungsmittelinduzierten Anaphylaxien im Kindesalter, z. B. von 41/100.000 Notfallaufnahmen im Jahr 2007 auf 72/100.000 im Jahr 2012 [9, 10].
Nahrungsmittel (z. B. Erdnüsse, Baumnüsse, Schalentiere), Insektengifte (z. B. von Honigbienen, Wespen) und Medikamente (z. B. Antibiotika, Schmerzmittel, Impfstoffe) gehören zu den häufigsten Auslösern einer Anaphylaxie [1, 2, 11]. Mögliche Anaphylaxien durch Impfstoffe wurden insbesondere in der COVID-19-Pandemie stark in der Öffentlichkeit diskutiert [12, 13, 14, 15, 16, 17]. Angesichts der zahlreichen Auslöser und der vielfältigen Möglichkeiten der Exposition im Laufe des Lebens ergeben sich Schätzungen von 0,3 bis 15 % Lebenszeitprävalenz von Anaphylaxien in der Bevölkerung, wobei allerdings auch leichtere Reaktionen wie eine exogen ausgelöste akute Urtikaria in manchen Studien eingeschlossen wurden [18, 19].
Die intramuskuläre (IM) Injektion von Adrenalin in den mittleren anterolateralen (AL) Oberschenkel ist die Behandlung der ersten Wahl bei Anaphylaxie (World Anaphylaxis Organization, WAO 2020) [1, 20, 21].
Die vordosierten Adrenalin-Autoinjektoren (AAI) sind einfach zu handhabende, selbstinjizierende Notfallgeräte, die eine prompte Adrenalinabgabe gewährleisten und somit eine lebensrettende Notfallbehandlung darstellen [2]. Sie vermeiden mögliche Dosierungsfehler (Über- oder Unterdosierung), die durch das manuelle Aufziehen von Adrenalin über eine Spritze oder das Zusammensetzen von Ampulle/Nadel/Spritze entstehen können, insbesondere im Stress einer Notfallsituation [22].
