Die Lipiodol-Lymphographie – Eine Spurensuche 1961-2019

Die Darstellung des menschlichen Lymphgefäßsystems durch intravasale Injektion des öligen Röntgen-Kontrastmittels Lipiodol UF® ermöglicht Einblicke in Struktur und Funktion von Lymphgefäßen und -knoten. Im Zeitraum, in dem es noch keine Computer-Tomographie (CT) und Magnet­resonanz-Tomographie (MRT) gab, war die Methode eine wertvolle diagnostische Bereicherung, insbesondere bei der Erfassung abnormaler Veränderungen an inguinalen, pelvinen, lumbalen und axillären Lymphknoten.

 

Nach Einführung der nicht invasiven Schnittbildverfahren Sonographie, CT und MRT sowie durch die Funktions-Lymphszintigraphie, die ICG-Fluoreszenz-Lymphographie und die indirekte Lymphangiographie hat die invasive Lipiodol-Lymphographie ihre ursprüngliche Bedeutung als diagnostische Routinemethode weitgehend verloren.

Gegenwärtig beschränkt sich die Anwendung der Methode sowohl in Form der alten intra­vasalen als auch der neuen intranodalen Technik der Lipiodol UF®-Infusion auf die Erkennung, Lokalisierung und Behandlung von posttraumatischen Lymphgefäßdefekten als Ursache von thorakalen und/oder abdominalen chylösen Erkrankungen. Bei dieser Gruppe von Krankheiten wird die Methode auch zur Lokalisierung der Cisterna chyli für nachfolgende interventionelle Maßnahmen verwendet.

Lipiodol lymphography – Tracing its history from 1961 to 2019

Summary
Visualization of the human lymphatic system by means of intravascular injection of the oily contrast agent Lipiodol UF® provides insights into the structure and function of lymphatic vessels and lymph nodes. During the pre-CT and MRI period prior to the 1980s, the method served as a valuable diagnostic tool, particularly for detecting abnormal changes in inguinal, pelvic, lumbar and axillary lymph nodes.

After the introduction of non-invasive cross-sectional imaging techniques such as ultrasound, computed tomography, and magnetic resonance imaging, as well as the lymphoscintigraphic function test, ICG fluorescence lymphography, and indirect lymphangiography, lipiodol lymphography as an invasive method largely lost its original significance as a valuable routine diagnostic method.

At present, both the old intravascular and the new intranodal technique of Lipiodol UF® injection is limited to the detection, location, and treatment of lymphatic vessel defects as a cause of thoracic and/or abdominal chylous disorders. In this group of diseases, the method is also used to locate the cisterna chyli for subsequent interventional procedures.
